HR Effectiveness Survey

The Human Resources/Payroll department wants to know how well we are serving our employees/internal customers. We very much would appreciate it if you would take the time to complete this survey. Please feel free to suggest improvements in any area.

Notes about the survey:

  • While a signature is not mandatory, providing your name or department will help us analyze the results and target areas for improvement.

I. GENERAL ITEMS

This section deals with the general level of service provided by the HR department. Please rate your satisfaction level in each area. Use a scale of 1 to 5 with 1 being “poor,” 3 being “satisfactory” and 5 being “excellent.” Use the area beneath each item to indicate how these areas could be improved.
