APPENDIX B
After Hours and On-call Drug and Alcohol Testing Protocol

Should the need arise for after hours (between 8:00 p.m. and 8:00 a.m., 7 days a week), reasonable cause, drug and/or alcohol testing, please follow the steps below.

1. The supervisor will page Promptcare staff member Mark Quimby at 360-1389.  If he is not available, contact Thomas Kuhn at 327-5543. If prompted, enter a telephone number at which the supervisor can be reached. These names and numbers are accurate as of 3/1/09 and will be updated if changes occur.
2. The Promptcare staff member will return the call and identify the Promptcare facility – East or West – at which the test will be conducted. 
   • Promptcare East is located at 326 Woodcrest Drive (the corner of East 3rd Street and Woodcrest Drive). Look for the IU/Rebound entrance.
   • Promptcare West is located at 3443 W. Third St. Bloomington, Indiana (Whitehall Crossing Shopping Center – Next to Fifth/Third Bank).

3. The Promptcare staff member will meet the employee and supervisor at the designated Promptcare facility.
4. Upon arrival, the Promptcare staff member will complete the appropriate Chain of Custody procedure and paperwork and collect the sample.
5. Upon completion of these steps, the employee and supervisor will be released.  Follow the guidelines in Step 8. of the procedure.
