Prescription Benefits Effective January 1, 2005

M-Plan HMO

Current participants will experience the following change:
- A new "OTC Select" prescription benefit will be added to cover certain over-the-counter drugs with a $4 copay when they are pre-authorized.
New enrollees effective January 1 will receive plan information including an ID card and prescription benefit information via U.S. mail at their home address in mid-January.

IU PPO $900 Deductible and IU PPO-Plus Plans

IU PPO $900 Deductible and IU PPO-Plus Medical Care Plan participants will have a new prescription benefit program administered through Anthem Prescription Management as a result of the Indiana Aggregate Prescription Purchasing Program (IAPPP).

Participants will receive a booklet about this new program via U.S. mail at their home address in mid-December. This booklet is also available online. It describes how to transfer existing prescriptions to Anthem and how to initiate new prescriptions. FAQs are also included to help employees understand and access their prescription drug benefits beginning January 1.

Blue Preferred Plan

Current participants will experience the following changes:
- Prescription non-formulary brand copay will increase to $30 at retail and $60 at mail.
- Non-sedating antihistamines (Allegra, Zyrtec, Clarinex) will no longer be covered.
New enrollees effective January 1 will receive plan information including an ID card and prescription benefit information via U.S. mail at their home address in mid-January. A booklet for new members is also available online.
