Health Risk Assessment

One of the most important first steps to health is understanding what's really going on in your body. It all begins with the HealthMedia® Succeed™ Health Risk Assessment. After completing the assessment (which takes 20-25 minutes to complete and consists of a confidential health history and questionnaire), you will receive a report with personalized results. And, depending on your needs, you will automatically be invited to participate in programs designed to help you lose weight, quit smoking, beat insomnia, care for back pain, and more.

Follow these steps to complete the HealthMedia® Success™ Health Risk Assessment:

After completing the assessment (which takes approximately 20-25 minutes to complete and consists of a confidential health history and questionnaire), you will receive a report with personalized results.

  1. Go to www.my.healthmedia.com.
  2. Click the “Sign Up” button.
  3. The Access Code is your first initial, last name, and last four digits of your Social Security Number. Don’t use hyphens when entering your access code. Do use a space(s) if it is part of your name in the IU system. Spouses of employees follow the same instructions as noted here.  
    • Example for Jane Smith: jsmith6789.
    • If your name is hyphenated as in Jane Smith-Jones, remove the hyphen and type the name in full as in jsmithjones6789.
    • Example for John Van Camp: The space in the middle of the last name is included, so his access code would be jvan camp6789.
  4. Click “Save and Continue.”

Important Information:

To return to your assessment or to the other online programs, follow steps 1 and 2 above, then click “Log In.”

Privacy note: Your specific health risk assessment information will never be released to your employer. This is addressed further on the “Authorization for Use and Disclosure of Information” page in the assessment.
