Transcripts and Grades

Request a Transcript

Official transcripts are available from the Office of the Registrar at a cost of $7. You can request a copy in person, by mail, or by fax.

In Person

  • Print and complete the Transcript Request Form and bring it to the Office of the Registrar in Hawthorn Hall 109. It will take us about 10 minutes to process your request.

By Mail

  • Print, complete, and sign the Transcript Request Form.
  • If you're paying by check or money order, make it payable to IU Northwest and write your Social Security number in the memo section.
  • If you're paying by credit card, print, complete, and sign the Transcript Request Fee Form, noting your method of payment (Visa, MasterCard, or Discover).
  • Send the form(s) and payment to:

By Fax

  • Print and complete the Transcript Request Form.
  • Print and complete the Transcript Request Fee Form, noting your method of payment (Visa, MasterCard, or Discover).
  • Sign and date both forms.
  • Fax to (219) 981-4200.
  • Call (219) 980-6817 to verify that we received your fax.

Use OneStart, the university's Web-based student information service, to view your official IU transcript.

Incomplete Grades

Incomplete grades are given only when course work is substantially completed and of passing quality. If a student does not complete course requirements within one year of receiving an incomplete, the incomplete is changed to a failing grade.

Past-Due Financial Accounts

The Office of the Bursar is responsible for the collection of university financial obligations. An account that is past due is encumbered by the Office of the Bursar. If your account is encumbered, you will not be able to obtain copies of your transcript.

Confidentiality of Student Records

The Family Educational Rights and Privacy Act affords students certain rights with respect to their education records.
