Master's Degree, Family Nurse Practitioner Program - Admissions Criteria

Ready to Apply?   Accepting Applications between November 1 and April 1

To accurately complete the application and facilitate smooth and timely processing, please make sure all of the following requested information is submitted:

  • First, complete the online application from the graduate office (you can do this online at IU Northwest Graduate Programs)
  • Second, complete the application materials below.

Checklist for Application Packet - (All items below must be submitted in your packet)

  • Essential Abilities Form
  • Drug Testing Permission Form
  • Copy of current, active, unencumbered Indiana RN licensure (and from any other state in which you hold licensure)
  • Official Transcripts from all post-secondary coursework
  • Criminal Background within the last 12 months completed per instructions below (you should have completed the appropriate steps so we can receive a copy of the criminal background check)
  • Personal statement essay attached to application -250 word essay required (disregard online application request for the 700 word essay)
  • Paper copy of your graduate application
  • Two professional references with one from a current manager (included in the online graduate application)
  • Current Resume
  • Proof of medical insurance
  • Computer proficiency form
  • The GRE (Graduate Readiness Examination) is not required for admission to the program.
