Financial Aid Forms
These forms are available online for submission or download. If you need help or have questions as you fill them out, contact our office.
- Free Application for Federal Student Aid - Federal Code 001815
- Fee courtesy form for employees, dependent children and spouse is available on-line.
- Complete the Free Application for Federal Student Aid (FAFSA). IU Northwest’s school code is 001815. You can complete the FAFSA online, or by returning a paper copy to the federal processor by mail. Paper copies will no longer be available in our office. To request a paper copy students can call the Federal Student Aid Information Center at 1-800-4-FED-AID.
- If you are admitted to IU Northwest and apply for financial aid by the March 10 priority date, your financial
aid notification should arrive by June.
- Interested in Federal Summer Financial Aid
- You must apply for admission by June 1st to meet certain scholarship deadlines.

Follow-Up Income/Resource Form
If you have been notified by the Office of Financial Aid that follow up information on your budget has been requested, print, complete and submit the Follow Up Income/Resource Form.

Special Circumstances Appeal Form - 2011-2012 Academic Year
Did your, or your family's financial circumstances change after you filed your FAFSA form? You can file a Special Circumstances Appeal by completing a Special Circumstances Form, and supplying all required documentation.

Cancellation/Adjustment Form, 2011-2012 Academic Year
If you would like to cancel or decrease financial aid that has already been awarded (such as a student loan), complete and submit this form. You can also use this form to request an increase in financial aid (depending on eligibility). For example, students who cancelled their financial aid in the fall semester and who wish to re-instate their financial aid for the spring semester may use this form.

This form is for degree seeking students at IU Northwest who plan to take one or more classes at another campus while enrolled at IU Northwest.
