A. Application Deadlines:
- Health Information Technology, Health Information Administration = April 1 deadline
- Radiography, Radiologic Sciences (Clinical/Management, Diagnostic Medical Sonography, Radiation Therapy Concentration) = January 15 deadline
B. University Admission:
Applicants who have never enrolled in an undergraduate degree program at any campus of Indiana University, must also file an application with the Indiana University Northwest (IUN) Office of Admissions. Previous enrollment in a non-degree program at one of the campuses is not sufficient. Courses cannot be transferred without applying to IUN.

C. Submitting Additional Transcript(s) Directly to Health Profession:
Applicants are required to submit a separate transcript from:
- Each college or university attended other than IU. This includes courses taken at a college or university while enrolled in high school.
- High school
It is not necessary to submit an "official" sealed copy of the transcripts. A legible photocopy or unofficial student copy will be accepted. Failure to provide required transcript(s) will make the application packet incomplete. INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED. See section on "Late Application Packets" for further information.
D. Completing The Application Data Sheet:
Step 1: Select Program - Check the appropriate box(es).
Step 2: Provide Information - Complete information should be provided. The applicant's "current address" will be used to confirm receipt of the application.
Step 3: Educational Background - Applicants are required to list last institutions attended.
Step 4: Certification - All applicants are required to complete the certification area and indicate they agree with the statement.
Step 5: Survey - Please respond to the two questions. For the Health Professions program use only.
E. Submitting A Complete Application Packet:
The following items must be submitted with each on-line application packet submitted:
- Copy of all additional required transcript(s).
Only completed application packets that are received by the program's application deadline will be forwarded to the program's admissions committee.
F. Late Application Packets:
Application packets postmarked or received after the program's deadline are considered late. Late applications are only considered if all on-time applications have been reviewed and a seat remains open in the program.
