Application Form 2003/04 - Due March 30, 2004

Application Form (rev. 2003/04) Instructions: The application should be computer generated using the format below: All sections must be completed for the proposal to be considered. Discussion and assistance with the faculty sponsor while writing the research proposal is expected. 4 copies and 1 original are to be submitted to Dr. Huysken in Marram 243
  The proposal should include the following elements:

1. Project Title

2. Applicant information

    Name:
    Year in School
    Anticipated graduation date
    Department:
    Phone #:

3. Faculty Sponsor

Indicate if the faculty sponsor will participate in the evaluation process, and if so, include their phone number.

4. Date of Application

5. Abstract
A brief description of project, maximum 100 words, worded so that a non-specialist can understand it.

6. Objective of Research (maximum 100 words, what do you plan to accomplish?)

7. Description of Proposed Research
This section should include background information and detail the methods/procedures to be employed. It is important to demonstrate that your research will follow a logical agenda of investigation. It is recognized that this is discipline specific. By following a logical agenda you will demonstrate that your initial ideas or thoughts will be purposely investigated.

Information and guidelines for travel are available at http://www.indiana.edu/~travel/

For travel, per diem days are $30/day max. while, mileage is $0.345/mile for the first 500 miles, and $0.17 for 501-3000 miles.

For equipment, you must state that the equipment will remain with the department and indicate whether equipment will be used up over the course of this investigation i.e. rental: otherwise it must remain with the department and cost sharing with the department must be demonstrated.
 

Give a breakdown cost for all supplies

A stipend of a maximum of $6.00/hr is allowed, but will only be considered if a research course and project in your department has already been completed or the majority of the work in the research course is completed. If you are unsure, please contact Dr. Huysken for clarification.
